(5) Uveitis with peripupillary membranectomy
Video demonstrates phacoemulsification with foldable IOL in complicated uveitic hypermature cataract with 360 degree posterior synechiae and seclusio pupillae.
A 52 years old male with complicated uveitic hypermature cataract with 360 degree posterior synechiae and seclusio pupillae was operated for cataract with phacoemulsification and foldable IOL implantation. After main and side port entries, the capsule was stained with trypan blue and with support of viscoelastic substance the thick peri-pupillary membrane was peeled off with forcep. The hypermature cataract was de- bulked and capsulorhexis was made with a Uttrata forceps and cystitome. Routine phacoemulsification was done and before emulsifying the last piece the foldable IOL was injected into the bag so as to prevent capsular rupture.
This video demonstrates the importance of synechiolysis, peri-pupillary membrane peeling and de-bulking of the hypermature cataract in managing complicated uveitic cataract. The idea of IOL scaffold was implemented so as to prevent posterior capsular rupture in an already complicated eye.
