(17) Injecting the injector

To describe a rare complication during cataract surgery where a piece of the injector was accidentally injected into the anterior chamber during intraocular lens implantation. After uneventful phacoemulsification for refractive lens exchange to manage high myopia, the foldable intraocular lens ( IOL) was loaded into the cartridge. During IOL implantation, a resistance was noticed. Then the plunger was repeatedly withdrawn to dislodge the IOL. After repeated attempts, the IOL came out of the cartridge together with a piece of the injector producing a large iatrogenic intraocular foreign body. The IOL was used as a scaffold to protect the posterior capsule, the incision was then enlarged and the large foreign body was safely removed from the eye.
