Purpose
This study investigates and compares the optical performance and safety of a novel endocapsular device used in combination with a multifocal intraocular lens versus the same multifocal IOL implanted alone.
Setting
ASUCA Eye Clinic, Sendai, Japan
Methods
In this single-center, non-randomized prospective study, 104 eyes underwent cataract surgery and implanted the novel endocapsular device in combination with a CNWTT IOL (Alcon Vision LLC, Fort Worth, TX, USA) (Group 1), while the same number of eyes were implanted with the multifocal IOL alone (Group 2). The study was conducted at the ASUCA eye clinic between May 2022 and March 2024. Preoperative, 1-week, 1-month, 3-month, and 6-month postoperative data was collected. The examination included monocular uncorrected and corrected visual acuity (distances: 5, 1, 0.7, 0.5, 0.4 and 0.3m), slit-lamp examination, root mean square values for aberrations, intraocular pressure (IOP), flare values, and endothelial cell density (ECD) count. Patients also underwent automated contrast sensitivity (10.2/7.1/4.5/2.9/1.8/1.1 degrees), contrast sensitivity with glare (10.2/7.1/4.5/2.9/1.8/1.1 degrees) and completed subjective questionnaires (VFQ-25 and Multifocal Intraocular Lens Questionnaire).
Results
The study showed that distance and intermediate visual acuity (5m and 70cm) were comparable between the two groups, while near visual acuity (30cm) was significantly better in Group 1 with the novel endocapsular device (p = 0.0033), as was uncorrected near vision (p = 0.046). Contrast sensitivity across all angles, both with and without glare, was significantly improved in Group 1, suggesting enhanced clarity in challenging conditions. Eyewear dependency was greater in Group 2 for intermediate and near distances (p = 0.011 and p = 0.0037, respectively). In the VFQ25 and Multifocal IOL questionnaires, Group 1 showed advantages in peripheral vision, near activities, and satisfaction with near and intermediate vision. Safety outcomes were within acceptable limits and comparable between the groups.
Conclusion
The novel endocapsular device with a multifocal IOL significantly improved near vision, contrast sensitivity, and reduced eyewear dependence, especially for near and intermediate tasks. The findings indicate that the novel device confers a meaningful improvement in quality of life for patients, as evidenced by enhanced peripheral visual function and greater satisfaction with near and intermediate-range visual performance.
