Purpose

Report and describe a case of twin sisters wth congenital cataract.

Setting

Two females twin 60 years old, wtih congenital cataract polar posterior, diagnosticated in the childhood without symptoms until early 2023 when referred blurred vision.

Methods

Both sisters was evaluated in ophthalmologist office with slith lamp (Imai), fundus camera (CSO cobra model), tonometry (Canon), autorefraction (Charops) and interferometry (IOL Master 700 Zeiss Meditec to IOL power) finding

a congenital polar posterior cataract in each one.

I decided performed a phacoemulsification surgery in every twin first one eye and next eye at the month.

The phacoemulsification was realized with technique "divide and conquer",  previous hydrodelamination (not hydrodissection) for the risk to do a posterior capsule rupture.

A one piece IOL monofocal was colocated into the bag without complications in both eye from the two sisters.

Results

To the twin one the IOL power in the right eye was +27.50 D (axial length 22.53 mm) and the left eye was +27.00 D  (axial length 22.39 mm) with residual refraction right eye -0.75 sph and lef eye -1.00 sph.

Visual Acuity: rigth eye 20/40  left eye: 20/30

Twin two the IOL power in the right eye was +27.00 D (axial length 22.43 mm) and the left eye was +26.50 D  (axial length 22.23mm) with residual refraction right eye -1.00 x 13º and lef eye -1.00 x 156º.

Visual Acuity: rigth eye 20/30  left eye: 20/25

Conclusion

The congenital polar posterior cataract is a rare entity and is convenient delay the surgical treatment, meanwhile check visual acuity and refreaction to avoid amblyopia.

When the cataract provokes blurred vision is indicated the surgery, now the phacoemulsification must be performed very slow and to do hydrodelamination (never hydrodisection).
