Purpose
To present the management of graft failure due to improper preservation technique.
Setting
Cornea and Transplantation department, Ophthalmology Clinic, ‘’G. Gennimatas’’ Athens General Hospital
Report of case
The case is about a 75-year-old male had been subjected to Descemet Stripping Endothelial Keratoplasty (DSAEK) a year prior at our Cornea and Transplantation department. Patient’s vision did not improve as the graft failed to function and a second DSAEK procedure took place. After the second surgery the patient recovered functional vision and we attributed the failure of the first graft to improper preservation technique.
Conclusion / Take home message
The providing agency of the graft preserved it in high concentration of amphotericin-C which proved to be detrimental to the endothelial cells. As the graft belonged to a young donor without ocular pathology and a great number of endothelial cells and was less than 10 days old, we conclude that amphotericin-C damages the endothelial cells rendering the grafts unable to improve vision.
