Purpose
Artificial iris implantation for traumatic mydriasis in an eye with scleral fixated Carlevale IOL
Setting
Department of Ophthalmology, NHS University Hospital, UK
Methods
The patient was previous implanted with a Carlevale scleral fixated IOL implant.
To ensure stable fixation of the implant, good centration and avoidance of any risk of future decentration or migration of the artificial iris implant, the implant was scleral sutured with the use of Gore-Tex CV8 sutures.
The Gore-tex sutures were preplaced on the iris implant, cut and externalised through sclerotomies with the use of MST forceps, after implantation of the device in the anterior chamber. The anterior chamber was maintained with the use of an AC maintainer.
This technique avoids the use of needles in the anterior chamber, limits the risk of endothelial damage and is easier to perform.
Results
The postoperative course was uneventful with clear cornea from day one after surgery.
Patient remains stable 12 months after surgery.
The artificial iris is well centred and the SFIOL remains stable.
The UCVA is 6/12 and BCVA is 6/9 with +1.00DS/-1.00@100.
The symptoms of glare and photophobia have fully resolved.
The patient is very satisfied with the excellent cosmesis and colour matching with the left eye.
Conclusion
We present the implantation of artificial iris, in an eye with previously implanted Carlevale SFIOL. To our knowledge, there is no previous such report.
Suture fixation provides good centration and stability of the iris implant with excellent functional outcome, no damage to the implanted Carlevale SFIOL and no complications.
