Purpose
To present a case in which we treated painful corneal edema in a patient with an eye lacking visual potential (NLP) using anterior stromal puncture and corneal collagen cross-linking.
Setting
The patient was followed up and treated at the Ophthalmology Department of Tel Aviv Sourasky Medical Center - Ichilov
Report of case
An 84-year-old man with no light perception due to branch retinal vein occlusion and glaucoma has been under follow-up at our clinic. He developed painful corneal edema and was subsequently treated with contact lenses for many years. However, he recently could not tolerate the lens and found it difficult to come for replacements. Thus, he inquired about alternative treatment. We opted to perform an anterior stromal puncture with corneal collagen cross-linking in order to alleviate his symptoms. After the treatment, the central corneal thickness decreased significantly from 760 µm to 570 µm and there was a significant improvement in the corneal edema. Additionally, the patient reported a marked improvement in pain and discomfort. A long follow-up of six months showed stable surgical results.
Conclusion / Take home message
This case demonstrates the significant relief of symptoms that may be achieved through a combined treatment of anterior stromal puncture and corneal collagen cross-linking, providing a long-term solution for corneal edema and pain.
