Purpose
The purpose of this study is to learn a technique of phacoemulsification with phaco machine to avoid posterior capsular rent and nucleus drop with second intrument or phaco probe and achieving endothelial safety within safe limits
Setting
I operated on 1000 patients with this technique and inclusion criteria was all types and stages of cataract including nuclear sclerosis ,posterior subcapsular cataract, mature and morgagnian cataract with 3 different phaco machines.
Methods
With few modifications in conventional method including capsulorrhexis of 6mm diameter and hydrodissection enough to bring the nucleus partially above the continuous curvilinear capsulorrhexis margin, followed by dialing and pbacoemulsification after putting generous amount of viscoelastic in anterior chamber to avoid endothelial damage.Maintaining phaco power not more than 60 and chopping the nucleus with phaco probe single handedly or with chopper.
Results
There is very less chance of posterior capsular rent in this technique if there would be any then nucleus can be revived through extending the incision and there is very less chance of it to be dropped posteriorly.
95% patients had 6/6-6/18 vision depeding upon other underlying conditions of retina.
Conclusion
The procedure with few modification is overall safe woth good post operative results and the learning curve is friendly with beginners as well as competent. This is quite productive in setups having no retinal surgeons and pars plana vitrectomy machine.
This is especially safe for new surveons learning the depth control.
