Purpose
Patients with pseudoexfoliation glaucoma are known to have limited response to medical treatment and faster progression, if untreated or medically uncontrolled. The purpose of this case is to present the effectiveness of combined phacoemulsification and goniotomy surgery in a patient with severe pseudoexfoliative glaucoma (XFG).
Setting
Ophthalmology Department, tertiary care hospital.
Methods
We present the case of a male 60 years old patient with advanced XFG and cataract in the left eye. Mean intraocular pressure (IOP) was 21±3 mmHg at baseline preoperatively on topical treatment with 3 IOP –lowering medications. Best visual acuity (BCVA) was 10/10cc. The patient underwent combined phaco-goniotomy surgery in the left eye.
Results
Postoperatively, in a short period of time, there was a 57% reduction in mean IOP (9±3 mmHg) on no medication. Meanwhile, the target IOP was achieved with reduction in medications (66,7% decline) , remaining <15mmHg. BCVA was 9/10sc and stable at last follow up.
Conclusion
In patients with cataract and advanced glaucoma, combined surgery with phacoemulsification and goniotomy is preferred to reduce IOP or number of anti-glaucoma medications or to prevent postoperative IOP spikes. Moreover, it does not adversely affect visual rehabilitation.
