Purpose
To find the safety & efficacy of TransPRK Combined with Corneal collagen crosslinking in patients keen for refractive surgery with thin cornea & other normal parameters.
Setting
STUDY DESIGN- Prospective Observational Study.
STUDY POPULATION- Walk in patients visiting our OPD for refractive surgery opinion.
SAMPLE SIZE- 20 eyes of 10 patients.
STUDY DURATION - 18 months.
INCLUSION CRITERIA-
Patients of age group more than 18 years.
Stable refraction (<5D Spherical equivalent) for more than 1 year.
Normal retina.
Central Corneal thickness above 450 microns.
Other all corneal topography parameters within normal limits.
EXCLUSION CRITERIA-
Established Keratoconus patients.
Keratoconus suspects.
Methods
The study included 20 eyes of 10 patients with spherical equivalent of -1 to -4.75 D.
The study was prospective.
Informed consent was obtained from each patient.
Follow up period was 12 months.
Machine used for excimer laser surface treatment - Schwind Amaris 750.
Follow up intervals were on 7th post op day, 1 month, 3 months and 1 year.
Results
Twenty eyes of 10 patients were included in our study. Follow-up duration was done on 7th post op day, 1 month, 3 months and 1 year.
Mean age at presentation was 26 years (range from 20 to 37 years).
2 were male and 18 were female.
Mean spherical equivalent (SE) refraction was −2.64 (standard deviation 0.965D, ranging from −1.00 D to −4.75 D)
Mean central corneal thickness was 469μm (SD 8.32μm, range from 455 to 485 μm).
At last follow-up, 18 out of 20 eyes had UDVA of 6/6. 2 out of 20 eyes had 6/9 and 6/7.5 UDVA. Those two eyes had postop SE refraction within -0.25D and rest all eyes had SE 0. None of the eyes lost BCVA postoperatively.
Conclusion
Combined Corneal collagen crosslinking & TransPRK procedure is very safe & effective method to perform on patients with physiological thin cornea.
This method maintained same visual outcome as with glasses in preoperative era , without any side effects in future. So it can be considered as a good refractive option in patients not suitable for other laser refractive procedures due to decreased corneal thickness.
