Purpose

To investigate the improvement in distance and near visual acuity in hyperopia and presbyopia patients after myopic lenticule transplantation, focusing on corneal shape modification (Q value) to ensure treatment efficacy without complications.

Setting

EYE HOSPITAL,KOSOVO

Report of case

A 45-year-old woman with hyperopia and presbyopia has been using glasses for both distance and near vision for five years.

Right cornea clear, CCT, 505 μm and UDVA, 0.1, CDVA, 1.0 +5.50 diopters and up to 13 mm Hg, UNVA, J 7, CNVA, J 2 +7.0.

Left cornea clear, CCT, 510 μm and UDVA, 0.2, CDVA, 1.0 +5.0 diopters, up to 14 mm Hg, UNVA, J7, CNVA, J2 +6.50 diopters.

A 2 mm incision in the right cornea, an intrastromal pocket of 8.0 mm in diameter was prepared with the Smile Module,

And a -6.0 myopic fresh lenticule was implanted.

A 2 mm incision and 8.0 intrastromal pockets were prepared in the left cornea and a -5.50 myopic lenticule was implanted.

Result

The patient was followed for three years. The patient had no complications, the cornea was clear, right and left UDVA, 1.0.

Right and left Preoperative, UNVA was J7 at 35 cm, improved to J2 after surgery. Similarly, preoperative UNVA was J6 at 70 cm and postoperative UNVA was J3 at 70 cm.

The patient no longer needed glasses for near and far vision after surgery.

Conclusion / Take home message

This method alters the cornea's shape to correct distance, intermediate, and near vision. Adding tissue instead of removing it reduces corneal instability and enhances correction for high hyperopia.
