To report the ocular drop contamination in 2 cases of infectious keratitis resulting in worsening of treatment in a tertiary case hospital in South india
Setting
Hospital based retropsective case series
Report of case
2 patients treated for fungal keratitis were followed up in cornea services . While on treatment both the patients showed sudden worsening of clinical symtoms and increase in corneal infiltrates. All the drops were subjected to microbiology.Topical medications cultured identified Pseudomonas aeruginosa. In addition one patient used a cotton bud to clean the bottle. One patient was repeatedly identified with drop contamination even after therapeutic keratoplasty.Proper antibiogram helped resolve both cases.
Conclusion / Take home message
It is important to suspect ocular medication contamination when patients show initial resilution and sudden worsening. Instruction on how to safely instill eye drops should not be overlooked.
