Purpose

Patients with Chronic Angle-Closure Glaucoma (CACG) have worse prognosis when they present with advanced disease and intraocular pressure (IOP) fluctuations despite topical medications. Lens extraction deepens the anterior chamber and widens the angle, causing decrease of IOP. In this case we present the effect of clear lens extraction in a patient with CACG.

Setting

Ophthalmology Department,tertiary care hospital.

Methods

Presentation of a 64 year old female patient diagnosed with CACG in both eyes under topical treatment  without known previous acute angle closure attacks. Slit-lamp examination showed narrow anterior chamber. Absence of peripheral anterior synechiae and visually significant cataract. IOP monitoring revealed fluctuations that remained uncontrolled with maximum medical therapy. Due to  rapid disease progression, it was decided to proceed to clear lens extraction (CLE) in left eye.

Results

Postoperatively, after 24hour and long lasting IOP monitoring , left eye presented mean IOP <19mmHg with reduced topical medications. Optical coherence tomography showed increased angle width. The patient reported without postoperative complications since the last follow up.

Conclusion

In agreement with the EAGLE study,CLE seemed to be efficient, causing widening of irodocorneal angle and reduce of mean IOP and required medications. It should be part of a shared decision making process between the clinician and the patient. Studies with larger number of participants and longer follow-up periods need to be done.
