Purpose

To evaluate the corneal endothelial cell changes after phacoemulsification in patients with COPD and compare the parameters with control group of similar age and sex using specular microscopy.

Setting

A prospective, hospital based case control study which was conducted in Ophthalmology Department in Minia University Hospital and Minia Eye Center (MEC) from February 2023 to September 2024.

Methods

This study include 100 cataractous eyes:

  • Group 1: Included 50 eyes of 50 patents with cataract and COPD.
  • Group 2: Included 50 eyes of 50 patients with cataract without COPD.

Inclusion criteria:

  • Age: more than 50 years.
  • Patient with cataract (nuclear not exceed N5, cortical not exceed C4 and post subcapsular not exceed P4 graded according to Lens Opacities Classification System III (LOCS III)) .
  • Patient with established diagnosis of COPD according to chest spesialist.

  •
  • Specular microscopy (Nidek,CEM-530):

Done 4 times; 1st preoperative and postoperative at 1, 3 and 6 months.

Results

Regarding the comparison of CD after 1 month, after 3 months and after 6 months postoperatively between the studied groups, the results were statistically significant (p value<0.05), as the mean CD was significantly lower among study group than control, while the mean CD was non-significant between study group and control preoperatively .

Regarding the comparison of CV after 1 month, after 3 months and after 6 months postoperatively between the studied groups, the results were statistically significant (p value<0.05), as mean CV was significantly lower among study group than control group, while the mean CV was non-significantly lower among study group than control group preoperatively (p value >0.05).

Regarding the comparison of preoperative HEX after 1 month, after 3 months and after 6 months postoperatively between the studied groups, the results were statistically significant (p value<0.05) as the mean HEX was significantly lower among study group than control group in all time periods

Conclusion

- The corneal endothelium is affected more in patients of COPD.

- Phacoemulsification should be done with more care in COPD patients than other patients.

- Patientsof COPD should be obsereved for longer time than other patients.
