Purpose

This study aimed to assess the refractive outcomes of commonly used modern formulas for IOL calculation in patients with mild Fuchs endothelial dtstrophy.

Setting

This is a retrospective study, conducted in one tertiary, university-affiliated medical center in central Israel.

Methods

This retrospective cohort study included patients who underwent corneal specular microscopy and biometric exams, with criteria including age > 18, corneal guttate, and corneal endothelial cell density between 1000 and 2500 cells/mm² and no corneal edema. Patients were matched with controls by age, sex, IOL model, and power. The study analyzed refractive outcomes using five IOL calculation formulas (Haigis, Holladay 1, Barrett Universal II, SRK/T and Kane). Statistical analysis was performed using R software, with results considered significant at p < 0.05. Ethics approval was granted.

Results

The study included 221 eyes, with 50 from patients with FECD and 171 from control patients. There were no significant differences in clinical or demographic characteristics between the two groups (P > 0.05). The postoperative spherical equivalent was -0.37 D in the Fuchs group and -0.30 D in the control group (P = 0.77). Both groups showed comparable standard deviations of the prediction error (PE) and absolute PE (APE) across all formulas (P > 0.05). In the Fuchs group, APE values were 0.34 D for Haigis, 0.32 D for Holladay1, 0.32 D for Barrett Universal II, 0.38 D for SRK/T, and 0.32 D for Kane, with no significant differences between formulas.

Conclusion

The prediction accuracy of IOL power calculations in patients with mild Fuchs was found to be consistent across various formulas and comparable to that in healthy controls. This suggests that IOL power calculations in mild Fuchs patients may be just as accurate and effective as in patients with healthy eyes, providing reassurance that the presence of guttate without edema does not compromise the reliability of these calculations.
