Results
A 35-year-old male initially presented with myopia (-4.5 OD and -7.75 OS) and corrected visual acuity of 10/10 in both eyes. Pre-operative corneal topography was normal, leading to the choice of wavefront-guided LASIK. Post-operatively, the patient had stable and satisfactory vision until 2019, when he developed progressive visual decline. At this time, refraction was -6 OD and -3.75 OS, with corrected visual acuities of 4/10 OD and 10/10 OS. Topography of the right eye showed inferotemporal steepening with increased curvature radii and localized thinning (pachymetry minimum 412 μm, Km 58.6). Epithelial mapping revealed inferotemporal epithelial hyperplasia with peripheral hypoplasia. Cross-linking and intracorneal ring segments were performed, resulting in improved visual acuity and ectasia stabilization within six months.
Conclusion
This case emphasizes the importance of long-term follow-up in post-LASIK patients, as late-onset ectasia can occur despite initially favorable outcomes, and timely intervention is crucial for preserving visual function.
