Purpose

The purpose of this study is to evaluate the safety and outcomes of same-day post-operative examination in routine cataract surgery patients at a large academic institution in the US.

Setting

Academic Institution

Methods

For qualifying routine topical phacoemulsification cataract surgeries, the patient’s first post-operative visit is conducted on the same day as their surgery, with a second visit conducted by the MD one week later. The study compares the primary endpoint of POW 1 and final visual acuity, along with various secondary endpoints such as post-op IOP, as well as the incidence of specific pre-defined post-operative complications. These are compared to literature reported averages for the same endpoints for the current standard of care of POD1 visits.

Results

Sample: Total of 4117 eyes,1659 of these Male, 2458 female. The mean age of the sample was 71.3.

Average POW1 VA = 0.35(POD0) and 0.379(POD1) ; p = 0.94 not significant

Percentage of patients with final BCVA >/= LogMAR 0.3 (20/40) = 96% (POD0) and 94% (POD1) p = 0.26 = not significant

Percentage of Patients with significant corneal edema – 1% (POD0) and 5.18% (POD1) ; p = 0.0085 + significant

Percentage of Patients with IOP > 25 = 35.4% (POD0) and 16.9% (POD1); p = .00009 + significant

Percentage of Patients with uveitis – 7.4% (POD0) and 3.29% (POD1) ; p = .001 + significant

Conclusion

Patients undergoing cataract surgery have similar outcomes with POD 0 and POD 1 cataract surgery follow up.

In our study there was a statistically significant difference in the amount of AC inflammation, corneal edema, and number of patients with elevated IOP >25. Despite this, the POW1 and final visual outcomes are similar between groups.
