Purpose
To evaluate the efficacy of Kahook dual blade in patients with uncontrolled glaucoma.
Setting
Ocular Surface and Glaucoma Service, Ophthalmology Dptm., Konstantopouleio-Patission Gen. Hptl., Athens, Greece, 2019-2022.
Methods
65 eyes of 62 patients with uncontrolled primary and secondary glaucoma, underwent ab-interno trabeculectomy with Kahook dual blade at an extent of 45-90 degrees, alone or in combination with phacoemulsification. The mean preoperative intraocular pressure (IOP) was 22.39±9.44 mmHg with 2.27±1.31 medications.
Results
All patients experienced minor bleeding at the end of the surgery. There were no other intraoperative or postoperative complications. The mean age of patients was 73.65±11.97 years. In the first month, the mean postoperative IOP was 17.26±6.59 mmHg with 2.37±0.91 drugs, while the final mean postoperative IOP was 14.17±3.80 mmHg with 2.33±0.87 drugs, mean follow-up at 10 months. In addition, a significantly (p=0.053) lower final IOP was observed in patients undergoing combined surgery (13.25±2.18 mmHg) compared to the other group (16.00±5.46 mmHg). The analysis by Pearson showed an association of higher preoperative IOP with higher postoperative IOP, with a strong indicator in the 3rd postoperative month (p<0.0001).
Conclusion
Kahook trabeculectomy is a safe and effective technique, reducing IOP by 36.71% and medications by 2.64%. It takes more patients and a longer follow-up period to confirm our initial results.
