Purpose

To study the incidence and pattern of dry eye post manual small incision cataract surgery(SICS)

Setting

Pre and post surgery prospective comparison of dry eye symptomatology and dry eye test scores of 82 eyes undergoing uneventful SICS at a rural center of our institute between August 2019 and January 2020 at post operative 1 week, 1 month and 3months followup using Tukey test.

Methods

Subjective symptoms of dry eye were scored using – OSDI questionnaire and functional evaluation of tear film done using Schirmers I, TBUT. NEI / Industrial grading system was used to score the ocular surface staining.

Above data was collected preoperatively, on postop 1 week, post op 1 month and at 3 months post surgery

Results

There’s a significant reduction in schirmers-1 scores at postop 1W -4.756 (95% CI: [-6.594, -2.918], p-value: 0.000), 1M -4.061 (95% CI: [-5.899, -2.223], p-value: 0.000) and 3 M -2.841 (95% CI: [-4.680, -1.003], p-value: 0.000)compared to preop.

There is a significant decrease in TBUT at 1 week -2.573 (95% CI: [-3.29, -1.856], p-value: 0.000) and 1 month postoperative-1.524 (95% CI: [-2.242, -0.807], p-value: 0.000) compared to preoperative levels, but this decrease is not significant at 3 months -0.5 (95% CI: [-1.217, 0.217], p-value: 0.438) implying improvement over time

There is a significant increase in Ocular Surface Disease Index (OSDI) scores at 1 week postoperative 13.298 (95% CI: [9.966, 16.631], p-value: 0.000),  no significant difference in OSDI scores at 1 month postoperative 1.848 (95% CI: [-1.485, 5.18], p-value: 0.928), while at 3M OSDI showed significant decrease compared to preoperative levels -3.57 (95% CI: [-6.902, -0.237], p-value: 0.036).

At POD3M, Inferior zone (18/22=81.81%) was the most commonly stained corneal zone followed by Central (14/22=63.63%). Staining noted in only 1 zone in 4 eyes, 2 zones in 8, 3 zones in 6 and in all 5 zones in 1 eye of the dry eye group

Conclusion

The dry eye values worsened in the immediate post op period at 1 weeek , while slowly ocular surface seemed to improve over 1 month and at 3 months post surgery. We reccomend throrough dry eye evaluation for patients with cataract  preoperatively and using tear supplements in the immediate post op period.
