Purpose

To assess the effectiveness of Small Incision Lenticule Extraction (SMILE) in improving visual acuity and patient satisfaction following trifocal intraocular lens (IOL) implantation.

Setting

Eye Hospital, Department of Refractive Surgery, Prishtina, Kosovo

Methods

This retrospective study involved 124 patients (162 eyes) who underwent cataract surgery with the implantation of the Zeiss AT Lisa tri 839MP intraocular lens (IOL). Preoperative myopic refraction varied between -1.0 and -2.75 diopters (D), while astigmatism ranged from -0.75 to -2.50 D. Following the SMILE procedure, all patients were monitored for one year. They completed questionnaires both prior to and one year after SMILE to evaluate their perceptions of vision and any vision-related difficulties. The protocol was registered on clinicaltrials.gov (NCT04693663).

Results

The uncorrected distance visual acuity increased from 0.66 ± 0.07 log MAR preoperatively to 0.07 ± 0.02 log MAR at 12 months postoperatively (p< 0.001). Patient satisfaction measures after SMILE significantly increased.

Conclusion

The study demonstrated that the SMILE effectively addresses residual refractive errors, a common issue following trifocal intraocular lens (IOL) implantation. This treatment modality was found to be both safe and effective in improving visual acuity in patients with pseudophakic myopic refractions.
