Purpose
The purpose of this study is to investigate a novel surgical method for corneal regeneration in patients with herpetic scarring. This method aims to enhance healing, restore corneal clarity, and improve visual outcomes while minimizing complications commonly associated with traditional techniques.
Setting
EYE HOSPITAL,PRISTINA,KOSOVO
Methods
The study included 23 eyes from 23 patients, aged 21 to 46 years, diagnosed with ocular herpes scarring. The thickness of the herpetic stromal scar was measured in microns using optical coherence tomography (OCT). The scar tissue extraction was performed using femtosecond laser small incision lenticule extraction (SMILE), followed by the transplantation of a fresh lenticule of the same volume through the incision. Lenticules were transplanted according to corneal topography.
All patients were followed for at least 3 years, with assessments including uncorrected distance visual acuity (UDVA), corrected distance visual acuity (CDVA), slit-lamp biomicroscopy, corneal topography, anterior segment optical coherence tomography and pachymetry.
A lenticule sample was obtained from a treated patient three years postoperatively for examination using transmission electron microscopy.
NCT05156151
Results
There was a significant improvement in preoperative corrected distance visual acuity (CDVA), increasing from 0.88 ± 0.26 logMAR to 0.49 ± 0.08 logMAR (P < .001). Additionally, the Q value decreased from -0.39 ± 0.04 preoperatively to -0.67 ± 0.05 postoperatively.
Conclusion
Lenticule transplantation is a safe and effective treatment that enhances visual acuity by modifying the Q value while facilitating corneal stabilization and regeneration.
