Purpose

To demonstrate a case of dead bag syndrome and the management of toric IOL misalignment

Setting

Private practice

Report of case

A 64 yo patient is seen 6 mo s/p uncomplicated phaco with implantation of a T4 monofocal IOL. It is 40 degrees misaligned with 1.75 D of postop astigmatism. Surgical rotation is attemped but is not possible due to dead bag syndrome. A CTR is eventually placed with the aid of capsule retractors and the IOL is eventually rotated into alignment. The new Shimowake CTR injector will be shown.

Conclusion / Take home message

Dead bag syndrome (DBS) is associated with zonulopathy and with toric IOL rotational instability. CTR injection is challenging with DBS
