Purpose
To assess the frequency of the changes in the anterior segment of the eye in patients with systemic lupus erythematosus and study their impact on patient’s life quality connected with vision.
Setting
35 patients hospitalized in the Rheumatology Unit were examined in the ophthalmological outpatient clinic of University Clinical Centre in Katowice. The study group contained 33 women and 2 men, average age was 49.1 years (range 22,5-68,5 years). The ophthalmological examination assessing anterior and posterior part of the eye with additional tests was performed. Based on the medical history and presented symptoms the most common ophthalmological symptoms in SLE patients were analized.
Methods
Ophthalmic examination covered best corrected and uncorrected visual acuity for near and far distances, intraocular pressure, examination of the anterior and posterior segment of the eye using slit lamp, Schirmer test, T-BUT test, Oxford staining, Bijsterveld’s corneal staining and OSDI questionnaire. Evaluation of the subjective quality of life related to vision using the EQ-5D-5L questionnaire and analysis of the activity of SLE according to the SLEDAI scale in comparison with the ophthalmic indexes of the dysfunction of the anterior segment of the eyesight were performed. All ophthalmic parameters were compared with clinical parameters (C3,C4 complement, protein level in blood, general symptoms of SLE) evaluating disease activity.
Results
The BCVA for the distance was on average 0.886 and 0.905 for the right and left eye in the group of the patients. We showed reduction in the total amount of water secreted in tears in patients using Schirmer Test (average - 11.34 mm / 5min), the break time of the tear film was TF-BUT 9.06. 37 % patients with SLE manifested DED based on Copenhagen and Japanese criterias. 17 patients achieved less than 23 points in OSDI questionnaire and 18 patients more than 23 points. Average ODSI was 24.2 points. The average negative correlation was (r = -0.3315) between the results of the EQ-VAS (Visual Analogue Scale) questionnaire and the results of the OSDI questionnaire (Ocular Surface Disease Index).
Conclusions
The pathologies of the anterior segment of the eye are more common in patients with systemic lupus erythematosus compared to population. Patients with SLE manifested ophthalmic signs and symptoms, mainly concerning anterior segment of the eye – keratoconjunctivitis sicca. The degree of advancement of systemic lupus erythematosus affects the progression of pathologies related to the vision. Changes in the anterior segment of the eyes that occur in patients with systemic lupus erythematosus reduce the quality of life related to vision. Life quality related to vision was lower in patients with SLE and DED what means treatment of ocular surface is important part of comprehensive treatment of patients with SLE.
