Cross-Linked Tectonic Corneal Lamellar Graft To Avoid Perforation

A 94-year-old man with history of age-related macular degeneration, glaucoma, pseudophakic, presented a infero-temporal limbal stromal defect greater than 50% of corneal thickness. At the biomicroscopic examination he had also infiltrate, keratic precipitates and no hipopion. Topical treatment with erythromycin ointment, quinolone eye drops, occlusion and oral treatment with doxycycline was prescribed. A blood test with autoimmunity profile was unremarkable. We first made a difficult conjunctival coating in that area because of adhesions and scleral thinning due to the intravitreal injections. When the sutures were removed there was a conjunctival retraction and progression of the limbal thinning. A second surgery was performed, an eccentric tectonic cross-linked lamellar keratoplasty and autologous conjunctival graft. A 4/4 punch was used. The surgical results were very positive. The patient was left without any pain or discomfort and maintains his visual acuity
The use of cross-linked corneal graft for the tectonic reconstruction of the anterior ocular segment represents an adequate surgical approach for the management of sclero-corneal melting.
