Purpose

Evaluate long-term clinical outcomes and assess the safety of combined pars plana vitrectomy (PPV) and scleral fixation of intraocular lens (IOL) Akreos AO60 using Gore-Tex suture.

Department of Ophthalmology, Centro Hospitalar Universitário de São João, Porto, Portugal.

Methods

Retrospective, interventional case series study. Primary outcome measures included change in best available visual acuity, intraoperative and postoperative complications. Patients subjected to combined PPV and scleral fixation of Baush and Lomb IOL Akreos AO60 with Gore-Tex suture between April 1st, 2017 and February 10th, 2022 with minimum follow-up of 30 days were included.

Results

Fifty-four eyes of 53 patients aging from 31 to 92 years old (mean 73 years) were identified. The mean follow-up period was 522 days (range 32-1564 days). Best available logMAR visual acuity improved from 1,32 preoperatively to 0,56 postoperatively (P<0.001). Intraoperative complications were reported in three eyes (5,6%), one iatrogenic retinal hole done during vitrectomy, a flat peripherical serous choroidal detachment and an intraoperative vitreous hemorrhage. Most frequent postoperative complications were transient corneal edema (n=16; 29,6%), transient ocular hypertension (n=13; 24,1%), cystoid macular edema (n=11; 20,4%) and transient hypotension (n=3; 5,6%). No suture related complications were observed.

Conclusions

Patients who underwent combined PPV and scleral fixation of Akreos AO60 IOL using Gore-Tex suture showed statistically significant improvement in visual acuity and no suture related complications, namely suture breaks, IOL displacements or suture-related inflammation. Gore-Tex scleral Akreos AO60 IOL fixations seem to be secure and stable in the evaluated time period. Immediate postoperative complications (corneal edema, hypertension and hypotension) may be frequent but are usually self-limited.
