Purpose

Extended depth of focus (EDOF) is a promising intraocular lens technology. This study aims to report the practical experience of implanting the EDOF AcrySof® IQ Vivity® (Alcon Inc., USA) IOL in refractive lens exchange patients using the mini-monovision concept.

Setting

The study is carried out in the SMILE-Eyes refractive-surgery center of the University Eye Clinic Marburg, Germany.

Methods

The study was designed as an interventional, prospective case series with a follow-up of at least 3 months. Patients who underwent refractive lens exchange and had bilateral implantation of the AcrySof® IQ Vivity® IOL are recruited. In all patients, a mini-monovision between 0.75 - 1.25 dpt anisometropia is aimed. We evaluate the refractive success of this IOL based on complete ophthalmologic examinations and performance of the Quality of Vision test ( IOL SAT Post-Operative Questionnaire and Catquest - 9SF). The main outcomes are the refractive outcome, distance, intermediate and near vision, and Quality of Vision Score.

Results

To date, 30 healthy eyes of 15 patients have been included (inclusion continuous). Best-corrected visual acuity remained the same with preOP -0.6 ± 0.7 LogMAR and postOp -0.4 ± 0.7 LogMAR (p = 0.5). Refractive outcomes were very good with UCDVA OU = - 0.01 ± 0.13, UCIVA O U= - 0.05 ± 0.12 and UCNVA OU = - 0.06 ± 0.13 at all distances. The most frequently complained visual disturbances were halos and glare, and this significantly more often by patients with monovision > 1dpt.

Conclusions

Implanted with monovision, the AcrySof® IQ Vivity® IOL provides good visual acuity at distance, intermediate distance, and near. Monovision > 0.75 dpt seems to increase the incidence of visual disturbances.
