Purpose

This paper presents a clinical case of enhancement after Smile, using Circle technique.

Setting

Methods

A typical case of myopic astigmatism that underwent a normal Smile procedure for correction, without any complication, in the dominant eye the refractive error was undercorrected, and the patient really disturb about his vision, as the nondominant eye was slightly amblyopic. After waiting and checking the results for an year, we decided to enhance. The Circle tehnique was performed, using a D pattern, the cap was transformed into a flap, with the hinge in a different position, in order not to superimpose with the initial SMILE incision.

Results

Postoperative refraction was closed to plano, after the enhancement. The Pentacam exam and corneal OCT were good, eye dominance changed the next day after the procedure, and the patient was very happy about his vision.

Conclusions

We have a good experience with SMILE, with many procedures performed so far. Enhacements after SMILE are rarely needed, and Circle proved to be a great technique, with good refractive results.
