Case Report – Unusual Case Of High Hyperopic Keratoconus

Purpose
We present a rare case of a high hyperopic patient with bilateral keratoconus.

Methods
38-year-old male with high hyperopia was evaluated for his visual acuity, refraction, slit-lamp eye examination, corneal tomography, epithelial thickness map and biometry
Results
Uncorrected visual acuity was OD 6/60, OS 6/30 and best corrected visual acuity was 6\8.5 in both eyes. The patient had high hyperopia and small astigmatism in both eyes (OD +9.75*-1.25*12, OS +8.75*-1.25*148). Refraction was stable for the last 7 years. Axial length was OD 21.83mm And OS 21.86mm. corneal tomography was compatible to bilateral keratoconus showing irregular astigmatism with inferior steepening in both eyes (Kmax = OD 48.2, OS 47.0), thinning of the cornea (OD 493 µm, OS 484µm) and posterior corneal elevation (OD +47µm and OS 46µm). The corneal epithelium was thinner at the steepest area of the cornea.
Conclusions
Although very rare, keratoconus could be found in patients with very high hyperopia. adequate visual acuity was achieved using glasses alone.
