Purpose

To assess preservation of globe integrity, visual outcome and maintenance of corneal clarity following corneoscleral graft transplantation for treatment of total anterior corneal staphyloma post resolved infective keratitis 

Setting

Memorial institute for ophthalmic research, Egypt

Methods

A retrospective study include 10 cases of 14 mm diameter corneoscleral graft transplantation done in cases of total anterior corneal staphyloma post resolved infective keratitis. Cases included were all having hand movement vision with good red/green light color discrimination. VEP and US were done in all cases preoperative. Cases were observed for preservation of globe integrity, visual outcome, intraocular pressure, and maintenance of corneal clarity one year after surgery.

Results

Preservation of globe were obtained in all the cases except one case, visual outcome improved to 0.1 in 3 cases and 0.05 in 2 cases and remain HM in 5 cases, glaucoma developed in 3 cases with glaucoma valve implantation needed in one of them, persistent hypotony occur in 3 cases. Graft clarity after one year was maintained in 8 cases, one case developed corneal rejection and one case developed persistent epithelial defect followed with corneal infection and needed conjunctival grafting.

Conclusions

Corneoscleral graft transplantation in cases of post infective anterior corneal staphyloma can preserve globe integrity and may regain vision but needs continuous follow up for IOP and signs of rejection
