Purpose

To describe the role of three- piece IOL with optic capture in microphthalmic eyes with congenital cataract.

Setting

Tertiary care hospital in North India.

Methods

A prospective study was conducted between January 2020 and January 2022 including 20 eyes of 10 patients with congenital or developmental cataract with microphthalmos. All children were less than 12 years of age. Children with white to white (WTW) greater than 9.5 mm and smaller than 8.5 mm were excluded. All children underwent lens aspiration with posterior curvilinear capsulorhexis and anterior vitrectomy, followed by insertion of three piece IOL in sulcus with optic capture.

Results

The mean age was 64 (+ 28) months. 60 percent of the participants were males. Majority (60%) of the children had a partially absorbed cataract. 10% had associated iridofundal coloboma. The mean WTW was 9.2 (+0.4)mm, sulcus to sulcus (STS) was 8.8 (+0.5) mm, and the bag diameter was 7.2 (+0.9) mm. Examination under anaesthesia at 3 months follow up revealed a well centered IOL on ultrasound bio microscopy in all kids. At 6 months follow up, none of the patients had glaucoma, corneal decompensation or anterior segment inflammation. None of the kids developed visual axis opacification.

Conclusions

Three-piece IOL with optic capture is a safe and viable option of IOL implantation in eligible microphthalmic eyes with cataract.
