Purpose

to assess subjective photic phenomena and spectacle dependence for different distances after bilateral implantation of different extended depth of focus (EDOF) IOL.

Setting

SI “Zaporizhzhia Medical Academy of Post-Graduate Education Ministry of Health of Ukraine”

Methods

41 patients (82 eyes) who had uncomplicated cataract surgery with bilateral implantation of different kind of EDOF IOL were enrolled. Patients were divided for groups by EDOF IOL type: monofocal EDOF (TECNIS Eyhance) – 15 subjects, non diffractive X-EDOF (AcrySof IQ Vivity) – 13 subjects, diffractive EDOF (TECNIS Symfony) – 6 persons, hybrid multifocal-EFOF IOL(TECNIS Synergy) – 7 persons.  Subjective photic phenomena and spectacle dependence for near, intermediate and far distance were studied. There were no difference in uncorrected distance and intermediate vision in all groups.

Results

Visual disturbances like halo were reported in 1 subject (7%) of both monofocal EDOF and X-EDOF groups only in the early postoperative period (1 month), that fully disappeared to the 6 month.  All patients with diffractive EDOF IOL noted photic phenomena like Halo (6 patients, 46%), Glare (7 subjects, 54%), Starbursts (2 patients, 15%) in the 1st month after surgery. There were reduction of visual disturbances at 6 month. Spectacle dependence for near vision was revealed at 12 persons (80%) with TECNIS Eyhance IOL, 3 person (50%) for TECNIS Symfony IOL, 2 patients (15%) for AcrySof IQ Vivity. Patients with  hybrid multifocal-EFOF IOL(TECNIS Synergy) reported fully spectacle independence.

Conclusions

High level of satisfaction in all groups of patients confirm that  each type of EDOF IOL must be selected into account according to the individual needs.
