Fenestrated Scleral Contact Lens To Improve Visual Outcomes In Post Penetrating Keratoplasty Patients

To study use of fenestrated scleral contact lens for improving visual outcomes in post penetrating keratoplasty patients
Case reported to the tertiary eye care centre in Andhra Pradesh state of south India
A 19 years old female, underwent PK due to infectious keratitis. Later on, for visual rehabilitation the patient was referred to CL clinic. RGP trial was done, and the patient was symptomatic, we gave her the option of Scleral contact lenses. Scleral lens improved her visual acuity from 20/80 to 20/20. Post 4 hours of lens use we observed significant corneal edema (>30 microns change) and visual acuity dropped to 20/40. To reduce corneal edema, Vault (distance between lens and cornea) and Material (high Dk) of the lens changed while ordering the lens but post 4 hours of lens use, showed corneal edema. To reduce the acute corneal edema, fenestration made on the lenses and corneal edema reduces to 9 microns post 4 hours of lens use and visual acuity remains same.
