Agreement And Reliability Of Iol Master Corneal Topography (Ct) Versus Pentacam Scheiumpflug Tomography In Determining Suitability For Premium Iol’S.

To assess the agreement and reliability of central topography (CT) of IOL Master 700 with that of Pentacam HR, as a screening tool for determining the suitability for premium ( toric monofocal, toric multifocal or multifocal implants – including EDOF) IOL implants.

A total of 200 eyes of eligible patients undergoing phacoemulsification with premium IOL’s were included in the study. IOL Master 700 CT and Pentacam scans were done preoperatively. These scans were reviewed by two experienced clincians. Both the observers were randomised using computer generated random numbers, to study one of the either scans at a time; and interpret it as Normal, Suspect or Abnormal pattern. Once the two observers finished seeing and interpreting the scan, their responses were enclosed in an envelope and forwarded to a blinded third observer for analysis of the responses. In case of a conflict, the clinicians interpreted the Pentacam scan together and came to a common interpretation which was then considered as final.

Of all the scans interpreted by the two observers, 77.5% (n=155) showed agreement whereas 22.5% (n=45) showed disagreement. The sensitivity of IOL Master CT 700 in diagnosing irregular corneas as not suitable for premium IOL implantation was found to be 72.73% while its specificity was found to be 78.09%. It’s Negative Predictive Value (NPV) was 95.86% and Positive Predictive Value (PPV) was 29.09%.

Results of our study suggest that IOL Master CT efficiently rules out irregular corneas while any suspicious scan should be confirmed with Pentacam HR for appropriate Premium IOL decision making.
