Descemetorhexis using different media: findings of the DMEK register, Linz

Descemet-Membrane-Endothelial-Keratoplasty (DMEK) is a great challenge for aspiring corneal surgeons as it has a relatively flat learning curve compared to other intraocular surgeries. This study analyzed retrospectively the separate surgical steps of all DMEKS that had been performed at Kepler University Eye Clinic Linz so far, and reviewed the outcome of the use of different media during descemetorhexis.

In total, 358 DMEKs performed by 3 surgeons from 2012 to 2017 were reviewed. In particular, the media used during descemetorhexis was analyzed. Significant differences in rebubbling rates were found. On average 0.49±0.73 rebubblings per patient were necessary. Out of all surgeries, 232 eyes (64.8%) had no rebubbling, 77 eyes (21.5%) one rebubbling and 49 eyes (13.7%) ≥2 rebubblings.


Although all 3 surgeons are experienced in DMEK, significant differences in the individual rebubbling rates could be found (p = 0.027). A statistically significant difference could also be noted in the comparison of different media and the number of rebubblings (p = 0.04). Healon GV® and air showed similarly low rates, whereas Healon® had higher rebubbling rates.


Especially in complex situations as deep-set eyes or large iris defects, Healon GV® seems to be an equally safe medium during descemetorhexis in DMEK. The authors assume that, due to its high surface tension, no Healon GV® remains between donor tissue and the recipient´s cornea when the anterior chamber has been flushed properly.
