Presbyopic refractive lens exchange with trifocal intraocular lens: refractive analysis of 35 206 eyes

To evaluate the short-term refractive results of presbyopic refractive lens exchange (RLE) with the implant of trifocal intra-ocular lens (IOL).

Retrospective interventional case series of 35 206 eyes submitted to presbyopic RLE with the implant of a single-platform trifocal IOL. The main outcomes measurements were composed of 3-months postoperative manifest refraction spherical equivalent (SEQ), astigmatism, Snellen UDVA, CDVA, UNVA, safety and efficacy indices.


From the total of eyes included, 7 789 (22.1%) eyes were previously myopic, 12 974 (36.9%) emmetropic and 14 443 (41.0%) hyperopic. At 3-months postoperative, 85.6% and 98.6% of the eyes were within 0.50D and 1.00D of SEQ, respectively. The manifest astigmatism was within 0.50D and 1.00D in 83.7% and 98.3%, respectively. The double-angle plot analysis demonstrated a centroid of 0.05D @ 85˚ ± 0.43D, with a mean absolute value of 0.28 ± 0.32D. The mean UDVA was 0.93 ± 0.19, with safety and efficacy indices of 0.96 ± 0.26 and 0.84 ± 0.26, respectively. The mean CDVA was 1.07 ± 0.14, with 92.8% of the eyes with loss of 0 lines, 4.13% loss of 1 line and 0.37% loss of 2 or more lines of CDVA.


Presbyopic refractive lens exchange is a safe and effective approach for the treatment of presbyopia. Long-term follow-up is important to evaluate the stability of the procedure.
