Comparison of pre-and postoperative peripapillary radial capillary plexus with optical coherence tomography angiography (OCT-A) in Preserflo patients

To compare peripapillary vessel density values calculated on optical coherence tomography angiography (OCT-A) images in primary open angle glaucoma patients before and after surgery.

In this prospective study, 30 eyes of 30 patients with POAG were included. Preserflo Microshunt was implanted in all cases in the sub-tenon space with mitomycin C 0.02% for 2 minutes. The peripapillary vessels were analyzed in superficial retinal layers from the radial peripapillary capillary segment (RPC) with OCT-A. Two groups of variables were measured, Small Vessels (SV) and ALL vessels, divided into Whole image, Inside disc and Peripapillary (total, superior and inferior). RCP density of the SV was also measured by hemispheres and quadrants, 16 variables were finally analyzed.


IOP decreased from 21,7± 6,3 to 10,5± 1,8 and 11±2 mmHg at 1 month and 3 months. 7 out of 10 variables measured in RPC Small Vessels and ALL Vessels increased from pre-op to the first month. By sectors, 5 out of the 6 variables of RPC SV also increased. At 3 months, 9 out of 10 increased and by sectors all of them increased except SV temporal. Mean total RPC increased from 37,37±5,98 to 38,12±6,20 (3 months). There was an statistically significant increase in the minimum mean value of RPC SV from preop to 3 months (20,92±7,34 to 27,02±6,81).


Signifficant IOP reduction in the early postoperative period after glaucoma surgery with Preserflo Microshunt induces an increase in vascularization in the radial peripapillary capillary segment, with a greater influence on the mean minimum value that increases significantly with lower IOP.
